Living as an elder should be an enjoyable part of your life. Why not enjoy yourself as part of a community that learns, plays and works together? At JABA, we have designed our Community Centers and our Adult Care Centers to promote fun and fellowship.
Support
JABA is all about lending support to seniors in need of assistance. Whether that includes basic information, case management, insurance counseling, home delivered meals — or any of the many other available services — we want you to think of JABA as a source of support for you or your loved ones. Most of our support is offered as a gift to those who cannot pay. See individual programs for more details.

JABA is able to do all that it does because of generous volunteers and donors. We value contributions of time as well as dollars and promise to put your talents and your donations to very good use.
In the past five years, government funding has diminished. Luckily, individual and corporate contributions have begun to rise. Your assistance is greatly appreciated and needed.

Case Management
JABA’s Case Management staff link individuals and their caregivers with vital services, which serve the best interest and well-being of each individual.
Our case managers work throughout the City of Charlottesville and the five- county surrounding area to ensure that seniors have access to the care that they need. Initial consultation is free. Fees for services vary and are based on income.
Any adult 60 years of age or more qualifies for the program. Request for case management may be made by the prospective client, the client’s family, or any individual or agency acting on the client’s behalf. JABA's Case Management Department focuses on providing personalized services to senior individuals residing on our community.
● This process begins with screening and assessment of the client and their social environment.
● Provision of services, both in-house and through community referrals takes place according to assessment, and is continued through ongoing evaluation of the client's progress.
● Case Management is intended to assist the client in obtaining and maintaining optimum functioning in the least restrictive environment while respecting the client's right to self-determination.

Case management is a collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individual's needs through available resources and to promote outcomes which:
- Respect a person’s individual choices
- Focus on maintaining the least restrictive environment
- Maximize an individual’s ability to remain independent
- Support caregivers
- Ensure changes are meaningful and sustainable
The Case Management Department at JABA focuses on delivering personalized services to seniors to improve their care and involves the following steps:
- Screening to determine the exact needs
- Developing of a plan of care specifically tailored for each person
- Coordinating the plan of care
- Evaluating the results and adjusting the care plan to meet the changing needs
Case managers practice person centered care which involves developing a plan based on what is important to a senior and acting on these beliefs (wishes) in conjunction with their caregiver(s). This model enables individuals to increase their personal self-determination and improve their independence by directing their own services.
