Case Management
JABA's Case Management service helps link individuals with the services they need and request. The case manager can help the client and his or her family by helping determine what kind of services, care or help are needed. Once that is determined, the case manager can help coordinate services, to ensure that the client is getting all of the help he or she needs. To start, the case manager meets with the client, and caregiver if appropriate, to assess needs, then creates a plan specifically tailored for that person. Then, with continued participation of client and family, the care manager can link the client and family to the right services, and can coordinate the implementation of the plan.
Qualifications
Any adult 60 years of age or more qualifies for the program. Request for case management may be made by the prospective client or client family directly or by any individual or agency acting on their behalf.
Cost
Initial consultation is free. Fees for services vary and are based on income. To learn more, schedule a consultation with a JABA case manager.

Care Coordination
Care Coordination provides a complete assessment of your needs and works with you to develop a plan to help you accomplish your goals. Care Coordination is available to those who do not qualify for Case Management because of the income guidelines, but who still want to take advantage of JABA's expertise in helping seniors. Care Coordination is a great option because it:
- Provides confidential, in-home assessment of needs.
- Helps determine your strengths as well as your needs.
- Links you to services you need and request.
- Provides direct access to services, including home safety and repair, home health and respite care.
- Coordinates medical appointments and other engagements as needed.
- Assists with transportation.
- Monitors and reports on progress.
- Provides support to local and long-distance caregivers.
- Advocates on your behalf for needed services.
You may call to request services. Your family member, friend, or caregiver may call to request services. The fees for these services are based on income, and set on an individual basis.
